Create a google form that allows Professors to request a change in the description of the course.
The validation of the request forms are to be manual for now
Create a link to that Google form from the YACS website to allow Professors to access the form at all
times
The current proposed place for the link is near the GITHUB link at the bottom of the YACS
homepage.

We should establish the exact contents of that form here and agree on them.
