Add table of contents to the left side of the slides

Based on the suggestion during the April 4 meeting with Christine and Patrick. Adding a simple table of contents with links to slides to the left of each page for easier navigation. Need input from @jeffersoncasimir and @christinerogers.

hi @iremkatitas - thanks for this. 2 questions:

• The PR preview link above doesn't work - can that be fixed, e.g. before @jeffersoncasimir goes on vacation ?
• I see a screenshot that looks like a fine start. is that a composite (like photoshopped) or is that what you have already, with workign links? (if so that's a solid start)

@christinerogers I believe the preview doesn't work because there's something wrong with the html code. The screenshot above is an edited image - not how I anticipate the menu to look but just to give an idea.
