Hi. I tested "Speak whole document" & "Speak current page" options & found that they never worked at all !
I perform:
setting | Accessibility | Speech | Engine
but "Engine" did not detect any thing when I tried it ......
Is this a permission problem or incorrectable limitation of flatpak package ?

I'm on Fedora Linux 30 X64 bit with espeakNG & eSpeak GStreamer multimedia playback both installed on my system ...... I test this issue after receiving last version of Okular: 1.10

Kindly, see the following screenshot:

As you see, no tts engine at all ! I tried to click it to see if something will be shown, but nothing !
Kindly, to fix this.

Doesn't work on 24.02.0. Prints this in logs:

Text-to-speech plug-in "speechd" is not supported.

@faveoled maybe try install "qt6-speech-speechd-plugin" package on Ubuntu, as KDE has switched to use Qt6, so this "qt6-speech-speechd-plugin" dependency is needed.

sudo apt install qt6-speech-speechd-plugin
