Form field inputs for UI config #3

If we could change some of the form fields in the UI config from their current components, that would be grand!

  • reportOnly: Checkbox (ideally this would be a Switch/Toggle, but alas we don't have that at our disposal)
  • reportUri: TextInput (leave as is)
  • unsafeEval: Checkbox
  • path: Textarea
  • excludedPath: Textarea

For the textareas, I'm thinking that the globs could be newline-separated. Thoughts?
