Using the `fountain-example` from `pygls` this commit adds the required boilerplate to stand up and run the basic `RstLanguageServer` inside `vscode.dev` Many features are not available and will be blocked until a mechanism for "file system" access can be figured out.

This is highly experimental but the vanilla docutils language server can now be run in the web version of VSCode!
There will be much to figure out before full Sphinx support can be enabled, but this at least gets the ball rolling.
