Attempt to adjust m2ts sample times to fix gaps in appended media

Samples in #5631 have segments with gaps that need to be adjusted. (cc @M-Joyce)
Or, decode times that appear to align perfectly, but still produce gaps (not resolved). (cc @tvinko)
