Civilization II v.2.42 not displaying dialog boxes properly in a new game with empty window running by side

When starting a new game under Civilization II with Conflicts in Civilization Scenarios and Fantastic Worlds expansions, the dialog boxes are not displaying properly with a blank window being launched with a C:\MPS\CIV2\CIV2.EXE title. The game also cannot be exited properly and has to be manually terminated via Task Manager.

The dialog boxes should display normally like the do under NTVDM or DOSBox running Windows 3.1x.
